Welcome to Outlive Concierge Medicine (OCM). By accessing or using our services, website, or patient portal, you agree to the following terms and conditions. Please read them carefully.
1. Medical Services Provided
Outlive Concierge Medicine offers concierge-style medical care, including preventive care, chronic disease management, hormone optimization, and healthspan-focused diagnostics. Our services are provided through telemedicine and in-person visits, depending on the patient’s location and clinical need.
OCM is not an insurance provider and does not bill insurance. Services are provided under a membership model. Emergencies and hospital care are not included.
2. Membership and Fees
Membership fees are billed monthly or annually as outlined on our [Pricing Page]. Fees are due in advance and auto-renew unless canceled in writing with appropriate notice.
Low-income or barter-based agreements may be considered on a case-by-case basis.
3. Cancellation and Refund Policy
Patients may cancel membership at any time with 30 days written notice. No prorated refunds will be given for the current billing cycle. Refunds may be considered in cases of billing errors.
4. Telehealth and Communication
By becoming a member, you consent to receive care via secure telehealth platforms, email, text messaging, and phone calls. While we use encrypted tools, no communication method is 100% secure. You understand and accept these risks.
OCM does not offer emergency care. In the event of a medical emergency, you agree to call 911 or go to the nearest emergency room.
5. SMS Communication Consent
By opting in, you may receive text messages related to:
- Appointment reminders
- Health updates or follow-ups
- Billing notifications
- Important announcements
1- SMS Consent Communication:
Information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
2- Types of SMS Communications:
If consent has been given to receive text messages from [Company Name], messages may be received related to the following (provide specific examples):

6. Privacy and Security
Your personal and medical information is protected under HIPAA. OCM uses HIPAA-compliant software for all patient communications and data storage. You agree not to share your login credentials or access your health information on shared or public devices.

7. No Insurance Reimbursement Guarantee
OCM does not bill insurance, Medicare, or Medicaid. While some patients may choose to submit invoices for potential HSA/FSA reimbursement, OCM does not guarantee reimbursement eligibility.
8. No Guarantee of Results
While OCM provides comprehensive care using evidence-informed strategies, results are not guaranteed. You understand that all medical treatment involves some degree of risk.
9. Limitation of Liability
To the fullest extent permitted by law, OCM and its providers are not liable for any damages arising from your use of services, except in cases of gross negligence or willful misconduct.
10. Governing Law
These Terms are governed by the laws of the State of Nevada. Any disputes will be resolved in Clark County, Nevada.
11. Changes to Terms
OCM reserves the right to modify these Terms at any time. Continued use of our services after changes are posted constitutes acceptance of those changes.
